The type of personal information we collect

We currently collect and process the following information:

·         Personal identifiers, contacts and characteristics (for example, name and contact details)

·         Your case history;

·         The assessment and reassessment of your health and health needs (including the outcomes of further investigations);

·         the initial and reviewed plans of care for you;

·         the care provided to you (including any advice given face to face or over the phone);

·         clinical images (if applicable).

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you for one of the following reasons:

·         Booking an appointment with ORKU Health Ltd either online, over the phone, or in-person;

·         Providing your Chiropractor with necessary information in order for them to provide the correct treatment or referral route;

·         For the purpose of signing-up to our mailing list.

We use the information that you have given us in order to

·         Book appointments and take payments;

·         Contact you about your appointments (e.g. sending reminders);

·         Provide you with treatment (including but not limited to Chiropractic Treatment, Massage and Rehabilitation classes such as Yoga);

·         Send you communications such as articles, offers, advice or newsletters relating to ORKU Health Ltd and the services offered (provided we have your consent to do so).

·         Refer to your GP, if necessary and consent is given

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

(a) Your consent

(b) We have a contractual obligation

(c) We have a legal obligation, in certain circumstances

(d) We have a legitimate interest

How we store your personal information

Patient records are stored electronically on specialist software for practice/patient management. This software is provided by PracticeHub.

We will also use other third-party software to enhance your care. This includes:

Rehab My Patient, which is used to send you details about rehabilitation exercises as part of your course of care.

CareResponse, which is used to gather feedback on your response to treatment and the improvements you have made throughout your time with ORKU. This is supported by The Royal College of Chiropractors

MailChimp, which is used to coordinate our marketing communication e-mails. Your name and email address will be held on their server

Patient records are stored safely and in good condition for eight years from the date of the patient’s last visit to ORKU Health Ltd or, if the patient is a child, until his or her 25th birthday, or 26th birthday if the patient was 17 at the conclusion of treatment.

The requirement of eight years is in line with the requirements that cover general NHS hospital records and other forms of health records. The purpose of this requirement is to make sure that the patient can have access to their recent health records and to provide protection for you if any complaints are made.

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

We will never share information with anyone who does not need access to your data without your written consent. The only people who will have routine access to your data are:

• Your practitioner so that they can provide you with care.

• Our reception team so that they can manage your appointments and assist in the provision of your care.
